Researchers are investigating the use of patient-derived glioblastoma organoids (GBOs) as models to predict how patients with primary or recurrent glioblastoma respond to conventional treatments like temozolomide and other repurposed drugs. The study aims to determine if drug responses seen in patients match those observed in their corresponding GBOs. This approach could help develop personalized treatment plans and explore new therapeutic options for this aggressive brain tumor, which currently has limited effective treatments. The study involves creating GBO lines from tumor samples collected during patients' standard treatment, which includes surgery followed by temozolomide-based chemoradiation therapy. These organoids undergo drug sensitivity testing to measure their response to temozolomide and other drugs using cell viability assays. The study retrospectively compares these results with patients' clinical outcomes like progression-free survival, without altering patients' current treatments based on organoid testing. Participants provide tumor samples and genetic information, while clinical outcomes such as progression-free and overall survival are tracked. Radiologic data are also collected prospectively. The main outcome measure is progression-free survival 12 months after surgery. This observational study does not involve direct treatment interventions and aims to validate GBO testing as a reliable preclinical platform that may eventually replace traditional clinical trials.
