Eligibility Criteria
You may qualify if you...
- Community Advisory Board members including health system, governmental, and community service stakeholders
- Able to understand English
- Adults over 18 years old
- General screening population: any patient who completes the SINCERE screener and can understand or read English or Spanish
- Survey study participants: patients over 18 years old who complete the SINCERE screener and have connected with United Way 211 for service outreach and can understand or read English or Spanish
You will not qualify if you...
- Unable to communicate verbally in English or Spanish
- Living in nursing facilities or not responsible for self-care
