Who Can Participate
Eligibility Criteria
You may qualify if you...
- Adults 18 years or older
- African American and/or Latinx
- Uncontrolled stage 2 hypertension, blood pressure 140/90 or higher
- Following with a Rush primary care provider in eligible Rush primary care clinics
- Has a smartphone (Android or iOS)
- Has an email address
You will not qualify if you...
- Organ transplant recipient
- On dialysis
- Already participating in another remote hypertension monitoring program
- Not interested in participating in the program
- Has already participated in the E3 Hypertension or E3 Diabetes program
