Eligibility Criteria

Eligible

You may qualify if you...

  • Adults 18 years or older
  • African American and/or Latinx
  • Uncontrolled stage 2 hypertension, blood pressure 140/90 or higher
  • Following with a Rush primary care provider in eligible Rush primary care clinics
  • Has a smartphone (Android or iOS)
  • Has an email address
Not Eligible

You will not qualify if you...

  • Organ transplant recipient
  • On dialysis
  • Already participating in another remote hypertension monitoring program
  • Not interested in participating in the program
  • Has already participated in the E3 Hypertension or E3 Diabetes program
