What this Trial Is About

Researchers are evaluating the potential benefits of inhaled nitroglycerin for patients with late-stage chronic obstructive pulmonary disease (COPD), particularly those experiencing respiratory failure. This study aims to investigate how nebulized nitroglycerin might improve pulmonary function by causing dilation of the pulmonary arteries and arterioles that are constricted in advanced COPD. This narrowing contributes to ventilation and blood flow mismatch and worsens the condition. The treatment might also enhance antibiotic delivery to the lungs, helping to better control infections. The study involves administering inhaled nitroglycerin to patients admitted to an adult intensive care unit with COPD exacerbation and respiratory failure. The treatment targets improving respiratory function by relieving pulmonary artery constriction. It is being studied in phases 2 and 3, focusing on its effects on pulmonary vasodilation and potential improvement of infection control in the lungs. The primary outcome measured is the change in pulmonary function five days after starting the nebulized nitroglycerin. Participants will be monitored closely after treatment begins to assess lung function changes and overall respiratory status. Researchers will use echocardiography to confirm pulmonary hypertension and will track pulmonary artery pressure and respiratory parameters. Safety will be observed, and pulmonary function tests will be performed after five days of treatment to evaluate the inhaled nitroglycerin's effects. The total study duration and follow-up details are focused on this early treatment period and respiratory outcomes.
