To demonstrate the safety and effectiveness of the Shockwave Reducer for treatment of patients with refractory angina pectoris treated with maximally tolerated guideline-directed medical therapy who demonstrate objective evidence of reversible myocardial ischemia in the distribution of the left coronary artery and who are deemed unsuitable for revascularization. A non-randomized single-arm registry will further assess the safety and effectiveness of the Shockwave Reducer in selected subjects with reversible myocardial ischemia in the distribution of the right coronary artery and who are deemed unsuitable for revascularization, subjects without documented obstructive coronary disease and abnormal coronary flow reserve (ANOCA), and subjects who cannot complete an exercise tolerance test due to lower limb amputation (above the ankle) or other physiologic condition with documented chronic mobility or balance issues that require the use of a walking aid.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Age older than 18 years
- Persistent refractory angina symptoms for 90 days or more classified as CCS Grade III or IV despite maximally tolerated guideline-directed medical therapy
- Tried maximally tolerated doses of at least three of four approved anti-anginal drug classes (long-acting nitrates, calcium channel blockers, beta blockers, ranolazine) with a stable regimen for at least 30 days prior to enrollment
- Deemed unsuitable or high risk for revascularization by coronary artery bypass grafting or percutaneous coronary intervention
- Evidence of reversible ischemia in the left coronary artery distribution by approved tests within 12 months prior to enrollment
- Functional limitation due to refractory angina with exercise tolerance test duration between 2 and 10 minutes on stable medication
- Left ventricular ejection fraction of 30% or higher within the past 12 months
- Willing and able to sign informed consent and comply with follow-up evaluations
- Three-vessel coronary angiography within 12 months showing obstructive coronary artery disease in the left coronary artery not suitable for revascularization
You will not qualify if you...
- Acute coronary syndrome with positive troponin or CKMB within 30 days prior to enrollment
- Successful revascularization by bypass surgery or stenting within 6 months prior to enrollment
- Unsuccessful PCI within 30 days prior to enrollment
- Predominant angina symptom is shortness of breath rather than chest pain
- Untreated hyperthyroidism, anemia with hemoglobin less than 10 g/dL, uncontrolled hypertension, rapid atrial fibrillation, severe aortic stenosis, hypertrophic cardiomyopathy with obstruction, or coronary artery vasospasm
- NYHA Class III or IV heart failure or hospitalization for heart failure within 90 days
- Life-threatening or implant-requiring heart rhythm disorders
- Severe chronic obstructive pulmonary disease or need for daytime oxygen or steroids
- Severe valvular heart disease
- Moderate or severe right ventricular dysfunction
- Presence of pacemaker lead in the coronary sinus
- Indication for implantable defibrillator or cardiac resynchronization therapy
- New pacemaker or defibrillator lead implantation within 90 days
- Severe kidney failure or dialysis
- Allergy to stainless steel or nickel
- Conditions interfering with protocol compliance or other investigational trial participation
- Pregnancy or planning pregnancy within 12 months
- Vulnerable populations unable to give consent
- Inability to tolerate required antiplatelet therapies
- Comorbidities limiting life expectancy to less than one year
- Current hospitalization for COVID-19 or recent symptomatic COVID-19 or positive COVID-19 test within specified timeframes
- Coronary anatomy suitable for revascularization with reasonable risk and likelihood of symptom relief
- High right atrial pressure or abnormal coronary sinus anatomy or unsuitable coronary sinus diameter as shown by angiogram
