What this Trial Is About
All participants of the Diabesity Register will receive structured assessment by nurses guided by the JADE portal at baseline including eye, feet, blood and urine examination with evaluation of BMI, blood pressure, HbA1c, lipid profile, renal and liver function tests, urine for albumin-creatinine ratio. All patients will return at 3-year for a repeat assessment of physical and psychological health, as well as changes over time.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Have type 2 diabetes
- Have obesity with body mass index (BMI) of 27.5 kg/m2 or higher and/or waist circumference of 80 cm or more in women or 90 cm or more in men
- Aged between 18 and 50 years
- Able to use smartphone technology and web-based programs
- Able to read and communicate in Chinese
You will not qualify if you...
- Have type 1 diabetes
- Have active malignant disease or history of malignant disease with less than 5 years disease-free
- Have life expectancy less than 12 months
- Have any medical condition considered by investigators to make participation unsuitable
