  • Patients prescribed physical rehabilitation for spine pain management
Not Eligible

You will not qualify if you...

  • Under 18 years of age
  • Over 89 years of age
  • Patients unable or medically unsafe to participate in physical rehabilitation for spine pain as determined by their physician
