Eligibility Criteria
You may qualify if you...
- Diagnosed with chronic heart failure according to ESC guidelines
- Age 18 years or older
- Admitted to hospital or had access to emergency care within the last year
- Own a smartphone (Android or iOS) capable of downloading and using the telemonitoring app
- Have Wi-Fi or 3G internet coverage at home
- Able to provide written informed consent
You will not qualify if you...
- Severe psychiatric disorder
- Unable to use portable technologies
- Unable to use the equipment provided
- Declines to give consent to participate in the study
