Eligibility Criteria

Eligible

You may qualify if you...

  • Diagnosis of idiopathic Parkinson's disease based on MDS criteria, including bradykinesia plus one other cardinal sign (resting tremor or rigidity), with no other known cause of parkinsonism
  • Modified Hoehn and Yahr stage less than 4
  • Age between 40 and 80 years at screening
  • Stable doses of PD symptomatic medications for at least 2 months prior to baseline, or not currently treated but unlikely to require medication in the next 6 months
Not Eligible

You will not qualify if you...

  • Diagnosis of atypical parkinsonism, drug-induced parkinsonism, essential tremor, primary dystonia, or other non-PD causes of symptoms
  • Significant neurological diseases other than PD that interfere with study procedures
  • Significant cognitive impairment (MoCA score below 23) or impairment preventing exercise adherence
  • Beck Depression Inventory II score above 16 indicating depression that prevents exercise
  • Use of neuroleptics/dopamine receptor blockers for over 30 days in the past year or within 30 days before baseline
  • Recent use of psychotropic medications without stable dosage for more than 30 days prior to screening
  • Known cardiovascular, metabolic, or renal disease or symptoms without medical clearance for exercise
  • Abnormal liver or renal function based on screening labs
  • Complete Blood Count abnormalities deemed clinically significant
  • Uncontrolled hypertension (resting blood pressure over 150/90 mmHg)
  • Orthostatic hypotension with standing systolic blood pressure below 100 mmHg
  • Already exercising 120 minutes or more per week at moderate intensity
  • Serious illness requiring systemic treatment or hospitalization within the last 4 weeks
  • Medical problems or injuries interfering with exercise ability
  • Conditions preventing safe lumbar puncture (e.g., coagulopathy, low platelet count, infection at puncture site, recent anticoagulant use except low dose aspirin, suspected hydrocephalus or intracranial mass, spinal disease)
  • Participation in another investigational study with intervention
  • Use of non-PD investigational products within 30 days before baseline
  • History of frequent falls or high fall risk
  • Lack of access to computer/tablet and WiFi or other technical issues preventing virtual exercise participation
