Who Can Participate
Eligibility Criteria
You may qualify if you...
- Age between 65 and 100 years old
- Living in the community
- Currently attending follow-up for chronic diseases at a polyclinic
- Able to communicate in English or Mandarin
- Able to understand the study and provide consent
You will not qualify if you...
- Currently receiving dietitian or physiotherapy intervention for special dietary or musculoskeletal conditions
- Living in an institution
- Having serious medical conditions that limit physical activity or high protein diet, such as severe heart disease, recent stroke, active cancer, or advanced kidney disease
- Terminal illness with life expectancy less than 12 months
- Severe hearing or vision impairment
- Unable to communicate in English or Mandarin
- Pregnant women
