Researchers are evaluating how a home-based exercise program combined with high protein diet counseling can help prevent frailty in elderly people living in Singapore. The study focuses on older adults aged 65 to 100 who have a mild level of frailty, identified by a clinical frailty scale score of 3 to 4. This trial takes place within Singapore's primary care setting, where patients receiving chronic disease care are screened and invited to participate. Participants will be randomly assigned to either an intervention group or a usual care group. Those in the intervention group will learn specific resistance and balance exercises through guidance and receive leaflets illustrating the home exercise routine. They will also get counseling about consuming enough protein as part of a balanced diet. Both groups will continue to receive standard primary care services, including dietitian and physiotherapy support if needed. Follow-up visits at 3 and 12 months will assess how well participants follow the exercise and diet program and provide reinforcement from care coordinators. Throughout the study, participants will attend three visits where various physical measurements such as weight, height, and hand grip strength will be taken. They will complete questionnaires about lifestyle, health conditions, quality of life, and frailty status. The main outcome measured is the Repeated Chair Rise Test, evaluated at baseline, 3 months, and 12 months. The study aims to monitor participants' adherence and the impact of the interventions over one year.
