The study aims to monitor participants' adherence and the impact of the interventions over one year.

You may qualify if you...

  • Age between 65 and 100 years old
  • Living in the community
  • Currently attending follow-up for chronic diseases at a polyclinic
  • Able to communicate in English or Mandarin
  • Able to understand the study and provide consent
Not Eligible

You will not qualify if you...

  • Currently receiving dietitian or physiotherapy intervention for special dietary or musculoskeletal conditions
  • Living in an institution
  • Having serious medical conditions that limit physical activity or high protein diet, such as severe heart disease, recent stroke, active cancer, or advanced kidney disease
  • Terminal illness with life expectancy less than 12 months
  • Severe hearing or vision impairment
  • Unable to communicate in English or Mandarin
  • Pregnant women
