Who Can Participate
Eligibility Criteria
You may qualify if you...
- Child younger than 18 years old at time of enrollment
- Hospitalized for pneumonia, skin and soft tissue infection, acute gastroenteritis, or urinary tract infection
- Parent or guardian speaks English or Spanish
You will not qualify if you...
- Has a chronic and complex medical condition
- Required surgical intervention beyond superficial incision and drainage for the main disease
- Has immunodeficiency
- Already has a well-child check-up or post-hospitalization follow-up visit scheduled within 7 days of discharge
- Parent or child strongly prefers either PRN or automatic follow-up
- Medical provider strongly recommends a follow-up visit within 7 days of discharge
- Has a sibling currently hospitalized
- No identified clinic for needed post-hospitalization follow-up
- Pneumonia with chest tube placement
- Urinary tract infection with neurogenic bladder, recent or planned urologic surgery, planned renal imaging within 7 days, or renal abscess
- Skin and soft tissue infection with chronic wound, postoperative infection, poor wound healing risk, discharge with drain, or complicated by necrotizing fasciitis or toxic shock syndrome
- Gastroenteritis complicated by hemolytic uremic syndrome
