Eligibility Criteria

Eligible

You may qualify if you...

  • Age 50 years or older
  • New hip fracture in orthopedic ward
  • Newly identified radiographic vertebral fractures in geriatric ward or clinical vertebral fractures in outpatient clinics
  • Newly prescribed with anti-osteoporosis medications (AOMs)
  • Recent change of AOMs
  • Poor adherence to AOMs
  • Physician believes patient will benefit from osteoporosis and medication management services

You will not qualify if you...

  • Life expectancy less than 2 years or communication problems that cannot be assessed
  • Atypical fractures
  • Participation in other medication clinical trials
  • Unwillingness to accept assessment and treatment
