Eligibility Criteria
You may qualify if you...
- Age 50 years or older
- New hip fracture in orthopedic ward
- Newly identified radiographic vertebral fractures in geriatric ward or clinical vertebral fractures in outpatient clinics
- Newly prescribed with anti-osteoporosis medications (AOMs)
- Recent change of AOMs
- Poor adherence to AOMs
- Physician believes patient will benefit from osteoporosis and medication management services
You will not qualify if you...
- Life expectancy less than 2 years or communication problems that cannot be assessed
- Atypical fractures
- Participation in other medication clinical trials
- Unwillingness to accept assessment and treatment
