What this Trial Is About
The objective of this study is to investigate the hypothesis that delivery at 35 0/7- 35 6/7 weeks in stable patients with gastroschisis is superior to observation and expectant management with a goal of delivery at 38 0/7 - 38 6/7 weeks. To test this hypothesis, we will complete a randomized, prospective, multi-institutional trial across NAFTNet-affiliated institutions. Patients may be enrolled in the study any time prior to 33 weeks, but will be randomized at 33 weeks to delivery at 35 weeks or observation with a goal of 38 weeks. The primary composite outcome will include stillbirth, neonatal death prior to discharge, respiratory morbidity, and need for parenteral nutrition at 30 days.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Speak English or Spanish
- Age 18 years or older
- Diagnosis of isolated fetal gastroschisis confirmed by ultrasound before 33 weeks of pregnancy
- Singleton pregnancy
- Able to provide written informed consent
- Established estimated due date before 22 weeks gestation by last menstrual period or ultrasound
You will not qualify if you...
- Fetal anomaly unrelated to gastroschisis such as chromosomal or structural abnormalities
- Severe fetal growth restriction below the 5th percentile for gestational age
- Maternal history of previous stillbirth
- Maternal history of spontaneous preterm delivery before 36 weeks
- Maternal cervical length less than 25 mm before 24 weeks if documented
- Maternal hypertension
- Maternal insulin-dependent diabetes
- Prenatal care started after 24 weeks of gestation
- Active COVID-19 infection not recovered by randomization
- Unstable pregnancy including abnormal amniotic fluid, abnormal umbilical artery Dopplers, or non-reassuring fetal tests
- Enrollment in another study affecting delivery or outcomes
- Use of surrogacy or gestational carrier
- Unable to provide informed consent or not legally authorized representative
