Evaluation of a New Care Model in Family Medicine Groups to Improve Medication and Support for Seniors with Major Neurocognitive Disorder

Researchers are evaluating a new model of care called the GPS project, designed to improve medication management and support autonomy for seniors with major neurocognitive disorder MCND or those undergoing cognitive assessment at home. The study aims to reduce the number of medications taken, avoid inappropriate prescriptions, and lessen the treatment burden while maintaining cognitive health and quality of life. This project also seeks to enhance satisfaction among seniors, their families, and healthcare professionals involved in their care. The GPS intervention involves nurses, doctors, pharmacists, and home care teams working together in Family Medicine Groups FMGs. Pharmacists conduct interviews with seniors and their caregivers to review medications thoroughly, reconcile prescriptions using reliable sources, and develop personalized care plans in collaboration with the healthcare team. About 200 seniors in FMGs receiving the GPS intervention will be compared to another 200 seniors receiving usual care in FMGs not using the GPS model. The study includes a 12-month recruitment period and approximately 6 months of follow-up for each participant. Participants will have their prescribed medications reviewed at the start, 3 months, and 6 months after joining the study to measure changes. Researchers will also assess changes in treatment burden from the start to the 6-month follow-up. The study involves no new drugs or devices but focuses on clinical interventions by pharmacists. Regular evaluations and follow-ups help monitor medication management, with the total participation time lasting about 6 months after enrollment.
