The model of care tested in the GPS project aims to optimize pharmacotherapy for seniors undergoing cognitive assessment or suffering from major neurocognitive disorder (MCND) at home. Who Can Participate
Eligibility Criteria
You may qualify if you...
- Seniors 65 years of age or older undergoing cognitive evaluation, referred to a memory clinic, or diagnosed with cognitive impairment within the last year or with major neurocognitive disorder and followed at home
- Referred to the pharmacist in participating Family Medicine Groups
- Currently taking prescription medications
You will not qualify if you...
- Seniors receiving palliative care
- Seniors unable to answer questionnaires in French and without a caregiver
