What this Trial Is About
People with high blood pressure and diabetes often have low levels of key nutrients as a result of not eating enough fresh fruits and vegetables. Research studies suggest that these nutrients can lower blood pressure and blood sugar. This study will investigate if providing encouragement to adopt this healthy and nutritious way of eating and providing guidance on weight management will lower blood pressure and blood sugar. The DASH-Life research study will recruit adults with hypertension and type 2 diabetes who live in Baltimore metro areas that don't have many stores selling healthy foods. The DASH-Life research study is 6 months long. Participants will be randomly assigned to one of two groups: 1) Self-Shopping DASH (S-DASH) diet advice group, or 2) Coaching DASH (C-DASH) diet advice group.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Age 18 years or older
- Diagnosis of type 2 diabetes by self-report, HbA1c6.5%, or diabetes medication use
- Baseline systolic blood pressure between 120 and 159 mmHg, with or without medications
- Serum potassium level greater than 3.0 and less than 5 mmol/L
- Estimated glomerular filtration rate (eGFR) of 45 mL/min/1.73m2 or higher
- Able and willing to complete study procedures in English
- Living in Baltimore areas classified as Healthy Food Priority Areas, USDA low income/low access areas, or census blocks with area deprivation index 60 or above (State Decile 6)
You will not qualify if you...
- Hospitalization or need for assistance due to hypoglycemia in the past 6 months
- Cardiovascular event within the past 6 months
- Use of diabetes or weight loss medications such as GLP-1, GLP-1/GIP, insulin, or combination therapies
- HbA1c level 10% or higher
- Diagnosis of type 1 diabetes
- Baseline systolic blood pressure less than 100 mmHg
- Weight over 400 pounds (due to scale limitations)
- Chronic diseases that could interfere with participation
- Current participation in other clinical trials affecting blood pressure, weight loss, diabetes, or adherence
- Participation in programs with conflicting dietary guidance like meal replacement or keto diets
- Unwillingness or inability to follow a DASH-like diet
- Planning to move away from the study area within 6 months
