What this Trial Is About
Researchers are evaluating a health advocate intervention designed to support pediatric patients who have undergone liver transplantation. This pilot study aims to determine how feasible and acceptable it is to integrate a Health Advocate into the transplant care team. The Health Advocate's role may include coordinating medical care, assisting with financial needs like taxi vouchers, and connecting families to community resources to improve self-management and communication between the healthcare system and families. The study involves a single group receiving the Health Advocate intervention, which focuses on addressing unmet social needs such as food insecurity, enhancing communication with healthcare providers, and improving care coordination. This behavioral intervention is tailored to pediatric liver transplant recipients and their families to support their ongoing care. Participants and their caregivers will be involved in assessments measuring changes in caregiver experience at the start and after 90 days. Researchers will also evaluate medication adherence, quality of interactions with the Health Advocate, and satisfaction with the intervention through surveys and qualitative feedback over 180 days. The total participation includes baseline and follow-up evaluations to monitor these outcomes and the intervention's impact.
