The study also evaluates the quality of interactions between the Health Advocate, healthcare teams, and community resources. Patient and provider satisfaction with the intervention will be assessed qualitatively. Participants engage through virtual visits enabled by a working phone or smart device capable of video or audio communication.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Patient has received a liver transplant more than 1 year ago
- Patient has been screened and considered likely to benefit from the Health Advocate program by the primary transplant practitioner
- Patient has an unmet social risk (such as food insecurity) or a Medication Level Variability Index (MLVI) of 2.0 or higher
- Patient will transfer to an adult transplant team within 6 months
- Patient can read and write English or Spanish
- Patient is between 18 and 25 years old and can provide informed consent
- Patient has a working phone and smart device capable of video or audio virtual visits via Zoom
You will not qualify if you...
- Patient has significant cognitive impairment
- Patient does not meet age criteria
- Patient is a ward of the state
- Patient does not speak English or Spanish
- Patient is not a US resident
