Participants will attend face-to-face and online sessions, complete questionnaires, log self-assessments, and undergo physical and health evaluations at baseline, post-intervention, and after follow-up. Outcomes measured include participant engagement, adherence, lifestyle behaviors, blood pressure, glucose, waist circumference, symptom burden, quality of life, depression, and physical function tests. Safety and adverse events are monitored throughout the 12-week intervention. The study involves interviews with some participants to gather feedback about the program's impact and design.

  • Adults aged 18 years or above
  • Diagnosed with cardiometabolic multimorbidity, defined as having two or more cardiometabolic diseases such as hypertension, type 2 diabetes, stroke, heart diseases, dyslipidemia, or obesity
  • Own a digital device with WeChat installed and have internet access
Not Eligible

You will not qualify if you...

  • Contraindications to exercise like severe musculoskeletal disorders, serious cardiovascular diseases, or spinal nerve injury
  • Diagnosis of psychiatric disease
  • Impaired cognitive function indicated by Abbreviated Mental Test Score of 6 or below
  • Impaired sensory or communication ability hindering participation, such as hearing loss, vision loss, or inability to speak Mandarin

Outpatient Treatment

Duration - 12 weeks

Participants in the intervention group receive a 12-week HcELP_CMM program consisting of two phases: Phase 1 includes a face-to-face goal-oriented health counseling session within one week after random allocation, and Phase 2 involves WeChat-based enhanced healthy lifestyle empowerment delivered weekly between weeks 2 and 12. Participants in the control group receive usual care including general health education on preventing and managing common diseases without the empowerment intervention.

Follow-up

Duration - 2 months
