What this Trial Is About
Researchers are evaluating the Health Chat for Empowerment-based Lifestyle Planning for Cardiometabolic Multimorbidity (HcELP_CMM) program in adults with cardiometabolic multimorbidity (CMM), which means having two or more cardiometabolic diseases such as hypertension, diabetes, stroke, heart conditions, dyslipidemia, or obesity. This study aims to assess if the program is feasible and acceptable, and to examine its short-term and long-term effects on lifestyle behaviors, heart and metabolic health indicators, symptom burden, quality of life, psychological well-being, and physical function compared to usual care. The study has two parts, both involving randomized assignment to either the HcELP_CMM intervention group or a usual care control group. The intervention lasts 12 weeks and includes two phases: first, an individualized, face-to-face goal-oriented health counseling session focusing on risk assessment, communication, goal setting, and commitment to lifestyle change; second, a WeChat-based empowerment program with weekly modules on lifestyle change and maintenance, self-assessments, bio-feedback monitoring, and bi-weekly online video counseling. The control group receives general health education without the empowerment components. Part 2 also includes a 2-month follow-up with WeChat-based check-ins. Participants will attend face-to-face and online sessions, complete questionnaires, log self-assessments, and undergo physical and health evaluations at baseline, post-intervention, and after follow-up. Outcomes measured include participant engagement, adherence, lifestyle behaviors, blood pressure, glucose, waist circumference, symptom burden, quality of life, depression, and physical function tests. Safety and adverse events are monitored throughout the 12-week intervention. The study involves interviews with some participants to gather feedback about the program's impact and design.
