If the HcELP\_CMM program has the potential to improve the lifestyle behaviors, cardiometabolic indicators, symptom burden, HRQoL, psychological well-being, and physical function in patients with CMM compared to the usual care group in the long-term?

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Adults aged 18 years or above
- Diagnosed with cardiometabolic multimorbidity (two or more cardiometabolic diseases such as primary hypertension, type 2 diabetes, stroke, heart diseases, dyslipidemia, and obesity)
- Have a digital device with WeChat installed and internet access
You will not qualify if you...
- Contraindications to exercise as defined by the American College of Sports Medicine, including severe musculoskeletal disorders, severe cardiovascular diseases, or spinal nerve injury
- Diagnosis of psychiatric disease
- Impaired cognitive function indicated by an Abbreviated Mental Test Score (AMTS) of 6 or less
- Impaired sensory or communication functions preventing participation, such as hearing loss, vision loss, or inability to speak Mandarin
