Who Can Participate
Eligibility Criteria
You may qualify if you...
- Age 18 or older
- Referred for cardiac rehabilitation for one of the following: acute myocardial infarction, percutaneous coronary intervention, coronary artery bypass surgery, heart valve repair or replacement, heart transplant, chronic stable angina, or chronic systolic heart failure (EF <=35%)
- Able to communicate in English, Spanish, or Cantonese
- Able to provide informed consent for self
You will not qualify if you...
- Currently enrolled in hospice care
- Provider advises that the patient should not be approached for participation in this study
