Eligibility Criteria

Eligible

You may qualify if you...

  • 18 years of age or older
  • English-speaking
  • Planned treatment with radical cystectomy or radical nephroureterectomy/ureterectomy with or without preceding systemic therapy
  • Enough time to complete at least 4 weeks of exercises before surgery if in the exercise group
  • Willing and able to participate in trial activities
Not Eligible

You will not qualify if you...

  • Cognitive or memory impairment preventing ability to follow exercise instructions
  • Immobility or inability/unwillingness to perform home exercise safely
  • Neurologic or orthopedic conditions restricting unsupervised home exercise
  • Nonmuscle-invasive urothelial cancer or metastatic disease including brain metastases
  • Predominant histology other than urothelial carcinoma not suitable for surgery
  • Uncontrolled or active illness limiting study compliance
  • Pregnancy
  • Inability to understand or read English
  • Lack of access to a compatible Android or iOS smart device
  • Not receiving surgery at University of Washington Medical Center
  • Participation in another clinical trial that prohibits enrollment in this study

Screening

Prehabilitation and Monitoring

Duration - 4 to 6 weeks before surgery

Participants begin at least 4 weeks prior to surgery. Arm A participants use the ExerciseRx app for four 20-30 minute home exercise sessions per week plus personalized step count goals, while Arm B participants receive standard educational materials. All participants wear a FitBit and use the FitBit app during this period while receiving standard chemotherapy if applicable.

Weekly remote monitoring visits

Duration - Surgery day

Participants undergo standard of care surgery for bladder or upper tract urothelial cancer following the prehabilitation or standard care period.

Post-surgery Follow-up

Duration - 90 days after surgery

Participants are followed for 90 days after surgery to assess recovery, physical function, and any complications. Arm A participants continue to use the ExerciseRx app and wear the FitBit during this time.
