Hospitals with clinical departments treating acutely ill inpatients that have or are ready to establish the prerequisites and infrastructure for Hospital-at-Home (HaH).
Hospitals must have or be ready to establish a governance structure to organize and oversee HaH services.
Hospitals must have or be ready to establish an IT platform capable of managing data to and from HaH patients.
Hospitals must have or be ready to establish a safe communication pathway between HaH patients and the hospital.
Hospitals must have or be ready to establish clear agreements on types of data to be exchanged between HaH patients and the hospital.
Hospitals must have or be ready to establish agreements on roles, responsibilities, and capacity, including possible primary sector entities.
Hospitals must have or be ready to establish clinical guidelines, standard operating procedures, and action plans to support HaH work.
