Eligibility Criteria

Eligible

You may qualify if you...

  • African-American adults who are patients at Neighborhood HealthSource in Minneapolis, Minnesota
  • Receive primary care at the partnering federally qualified health centers (FQHCs) and plan to continue care there for the next 12 months
  • Men and women aged 18 years or older
  • Uncontrolled hypertension defined as stage 2 hypertension with blood pressure of 140/90 mmHg or higher at most recent outpatient evaluation, with or without medications
  • Documented diagnosis of hypertension in electronic health records
  • At least one office visit at the partnering FQHCs in the past year
  • Own a smartphone supporting iOS or Android systems
  • Have an active email address and willingness to use it during the study
  • Basic internet navigation skills
  • Access to the internet at least weekly (e.g., at home, family or friend's home, church, library/community center, school/university, internet café)
Not Eligible

You will not qualify if you...

  • Diagnosis of a serious medical condition or disability that would make participation difficult, such as visual or hearing impairment or mental disability precluding independent app use
  • No primary care provider at the partnering FQHC
