Who Can Participate
Eligibility Criteria
You may qualify if you...
- African-American adults who are patients at Neighborhood HealthSource in Minneapolis, Minnesota
- Receive primary care at the partnering federally qualified health centers (FQHCs) and plan to continue care there for the next 12 months
- Men and women aged 18 years or older
- Uncontrolled hypertension defined as stage 2 hypertension with blood pressure of 140/90 mmHg or higher at most recent outpatient evaluation, with or without medications
- Documented diagnosis of hypertension in electronic health records
- At least one office visit at the partnering FQHCs in the past year
- Own a smartphone supporting iOS or Android systems
- Have an active email address and willingness to use it during the study
- Basic internet navigation skills
- Access to the internet at least weekly (e.g., at home, family or friend's home, church, library/community center, school/university, internet café)
You will not qualify if you...
- Diagnosis of a serious medical condition or disability that would make participation difficult, such as visual or hearing impairment or mental disability precluding independent app use
- No primary care provider at the partnering FQHC
