What this Trial Is About
Functional abdominal pain disorders (FAPDs) affect many children, with about 14% experiencing these pains, which have increased in recent years in Sweden. Children with FAPDs often face reduced quality of life, missed school days, and a notable portion also have psychiatric conditions. This research investigates how children's pain regulation differs between those with FAPDs and healthy controls, and examines which aspects of pain regulation relate to improvement during Internet-delivered cognitive behavioral therapy (Internet-CBT). The study evaluates an Internet-CBT intervention consisting of ten weekly modules for children and their parents, delivered online with asynchronous psychologist support via text messages. Participants include children aged 8 to 17 years with FAPDs receiving this treatment, and a control group of healthy children without persistent abdominal pain. The study compares pain regulation and other measures before and after a 10-week treatment or waiting period for healthy controls. Participants will undergo assessments such as pressure pain thresholds on the navel and thigh, unpleasantness ratings of pressure pain, conditioned pain modulation, temporal summation, sensory attenuation of pain, and various brain imaging scans using fMRI. Additional evaluations include questionnaires on gastrointestinal symptoms, pain rating scales, and behavioral responses related to irritable bowel syndrome. These measurements occur at baseline and after the 10-week period to understand treatment effects and pain regulation changes in children with FAPDs.
