The purpose of this trial is to evaluate the clinical efficacy, the cost-effectiveness and the effect durability of a therapist-guided, internet-delivered cognitive-behavior therapy intervention for obsessive-compulsive disorder (OCD) in children and adolescents with autism. A process evaluation of the treatment will also be conducted.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- A diagnosis of autism based on DSM-IV, DSM-5, or ICD-10 criteria, confirmed by validated instruments or medical records.
- A confirmed DSM-5 diagnosis of obsessive-compulsive disorder (OCD).
- A total score of 16 or higher on the Children's Yale-Brown Obsessive Compulsive Scale (CY-BOCS).
- Age between 7 and 17 years.
- Ability to read and write Swedish.
- Regular access to a computer or smartphone/tablet with internet and a mobile phone for text messages.
- A parent or caregiver able to participate in the treatment alongside the child.
You will not qualify if you...
- Global intellectual disability confirmed by medical records and intelligence testing.
- Comorbid psychotic disorder, bipolar disorder, severe eating disorder, severe depression, alcohol or substance dependence, or hoarding disorder.
- Current suicidal intent or suicide attempt within the past 12 months.
- Primary symptom presentation of hoarding symptoms.
- Completed cognitive-behavioral therapy (CBT) for OCD within the last 12 months (at least 5 sessions including exposure and response prevention).
- Receiving psychological treatment for OCD or anxiety at the same time.
- Medication changes for OCD or autism behavioral symptoms within 6 weeks prior to baseline.
- Having a close relative already participating in the study to avoid treatment group contamination.
