Who Can Participate
Eligibility Criteria
You may qualify if you...
- Patient or representative has given consent and signed the consent form
- Patient is affiliated with a health insurance program
- Patient is at least 65 years old
- Patient is treated with at least five medications for at least six months
- Patient is available for a 3-month follow-up
- Patient is hospitalized in the surgery department
- Patient has a Trivalle score greater than or equal to 2
- Patient lives in a nursing home or will return home after hospitalization

- Participating in another category I interventional study
- In an exclusion period determined by another study
- Under safeguard of justice
- Unable to receive informed information
- Receiving palliative care
