This research aims to implement and evaluate the Pediatric Integrated Nutrition Pathway for Acute Care (P-INPAC), an evidence-based algorithm designed to improve nutrition care for hospitalized children at risk of malnutrition. The study is a pilot conducted at three Canadian pediatric hospitals to assess feasibility and staff training effectiveness. It focuses on malnutrition identification, assessment, and early intervention to enhance routine nutrition care in pediatric wards. The study involves three phases. Phase 1 includes auditing health records of all hospitalized patients at selected wards every other week for 8 weeks to track existing nutrition care practices. A subgroup of patients will receive detailed malnutrition screening and assessment, with follow-up one month post-discharge. Healthcare staff knowledge and workload are also assessed. Phase 2 trains ward staff on P-INPAC procedures, including nutrition screening and the Subjective Global Nutrition Assessment. Phase 3 monitors implementation over 4 months, auditing patient care changes and reassessing staff knowledge and dietitian workload. Participants will undergo nutrition risk screenings using tools like STRONGkids and PNST, along with measurements such as weight, height, and head circumference. Healthcare providers complete knowledge and attitude questionnaires before and after implementation. Detailed patient data and follow-ups are collected to assess nutrition care improvements. The study measures acceptability, adoption, and coverage of nutrition practices, with participation lasting through the study phases including post-discharge follow-up.

1 to 2 visits depending on cohort assignment
Duration - 2 months for initial audit and 4 months post-implementation
Participants' routine nutrition care is audited on selected days to track nutrition care practices before and after implementation of P-INPAC.
One day of every other week for up to 6 months
Duration - 2 months for initial detailed assessments plus 4 months post-implementation
Participants in the detailed patient group receive malnutrition screening and assessment including the Subjective Global Nutrition Assessment (SGNA).
1 baseline visit and 1 follow-up visit one month after discharge
Duration - 1 month after hospital discharge
Participants are followed up one month after discharge to assess nutrition status and care outcomes.
1 follow-up visit (phone call) one month after discharge
