Eligibility Criteria
You may qualify if you...
- Adults with type 2 diabetes (T2D)
- Received primary care within the healthcare system with a primary care provider visit in the past 24 months
- No eGFR and/or urine albumin-creatinine ratio (UACR) test recorded in the electronic health record within the last 15 months
You will not qualify if you...
- Diagnosis of chronic kidney disease (CKD)
