Researchers are evaluating the effectiveness of Screening, Brief Intervention, and Referral to Treatment (SBIRT) among adults who show risky alcohol or substance use behaviors while receiving care at sexual and reproductive health clinics. The study aims to understand how SBIRT impacts alcohol and substance use, sexual and reproductive health, mental and physical health, quality of life, and wellbeing. It also examines whether SBIRT's effects differ by ethnicity, socioeconomic status, age, gender, urban versus rural settings, and delivery mode (in-person versus telemedicine). Participants are randomly assigned to either receive the SBIRT intervention or usual care. SBIRT involves validated screening tools followed by a brief motivational interview lasting 5 to 10 minutes, providing feedback and support to motivate change. Those with more severe substance use may receive referrals to specialty addiction treatment. The intervention is delivered both in-person and through telemedicine. Usual care varies by provider and generally lacks standardized screening or follow-up. Participants complete surveys at the start, 30 days, and 3 months to track outcomes such as alcohol and drug use, sexual behaviors, mental health symptoms, and overall wellbeing. Researchers also monitor the number of brief interventions delivered, provider and clinic use of SBIRT, and competency scores during preparation and implementation phases. The study includes a 12-month implementation phase and a 12-month sustainment phase to assess ongoing use and effects.

Duration - Up to 3 months
Participants who screen positive receive the Screening, Brief Intervention, and Referral to Treatment (SBIRT) intervention, delivered either in-person or through telemedicine. This includes a brief motivational interviewing-based session lasting 5-10 minutes and, if needed, referral to specialty addiction treatment.
1 baseline visit and follow-up assessments at Day 30 and Month 3
