Eligibility Criteria

Eligible

You may qualify if you...

  • Age 65 years and above
  • Diagnosis of cancer and completed chemotherapy with curative intent
  • Scheduled to receive a survivorship care plan from an oncology navigator
Not Eligible

You will not qualify if you...

  • Advanced dementia or serious cognitive impairment that impedes ability to participate in the interview
  • Nursing home resident
