Eligibility Criteria
You may qualify if you...
- Age 65 years and above
- Diagnosis of cancer and completed chemotherapy with curative intent
- Scheduled to receive a survivorship care plan from an oncology navigator
You will not qualify if you...
- Advanced dementia or serious cognitive impairment that impedes ability to participate in the interview
- Nursing home resident
