Eligibility Criteria

Eligible

You may qualify if you...

  • Age 13 to 21 years
  • Ask Suicide Questions (ASQ) screen score of 1-5 with no current plan or intent requiring acute referral
  • Risk level of suicidal thoughts and behaviors needing higher level assessment or safety plan as determined by a healthcare provider
  • One parent or guardian per adolescent will be invited to participate
  • All healthcare providers conducting assessments of suicidal ideation in participating clinics will be invited
  • All subjects must have internet access and a computer or tablet device
Not Eligible

You will not qualify if you...

  • Concurrent enrollment in an affiliated Suicide Care Research Center study
  • No phone access
  • Non-English speaking
