The study plans to enroll up to 3200 patients, with randomization ending by December 31, 2025. Participation involves receiving invitations via text, email, or phone calls and continuing routine care as usual.

Eligibility Criteria
You may qualify if you...
- 18 years or older
- At least 4 Geisinger emergency department visits in the last 6 months
- Member of Geisinger Health Plan (GHP) insurance
- Has a Geisinger Primary Care Provider (PCP)
- Receiving care at a participating Geisinger facility such as Geisinger Lewistown Hospital, Geisinger Medical Center, or others listed in the criteria
You will not qualify if you...
- Receiving care in departments deemed ineligible by the study team
- Diagnosed with alcohol and substance-related disorders
- Scheduled to receive inpatient care
- Coded as Level 1 in the emergency department (seeking life-saving measures)
- Has Intellectual Disability or Traumatic Brain Injury
- Currently seeing a behavioral health provider

Participants receive short-term bridge support through an integrated telemedicine program from a virtual team including a behavioral health provider and a Community Health Worker.
Regular virtual visits as part of the telemedicine program
Duration - Monitoring continues after the telemedicine program ends up to 120 days from initial enrollment
Participants transition from bridge support to ongoing care with their Primary Care Provider and Behavioral Health Provider, with monitoring of emergency department visits.
Additional follow-up visits depending on care needs
