What this Trial Is About

The trial investigates a virtual, on-demand telemedicine program designed for patients who frequently visit the emergency department (ED) and have both physical health conditions and behavioral health concerns. The purpose is to evaluate whether this integrated telemedicine program can reduce the number of ED visits within 120 days after the first alert. Researchers will compare this program to standard care to assess its impact on ED utilization. Participants will be randomly assigned to either the treatment group, which receives full access to the integrated telemedicine program, or the control group, which continues with standard care without the telemedicine intervention. The telemedicine program offers short-term support from a virtual team including behavioral health providers and community health workers. Once patients stabilize, they transition to ongoing care with primary care and behavioral health providers within the Geisinger Health System. During the study, participants will be monitored for ED visits within 120 days after their first alert to the program. Researchers will collect data on emergency department utilization and analyze results using an intent-to-treat approach. The study plans to enroll up to 3200 patients, with randomization ending by December 31, 2025. Participation involves receiving invitations via text, email, or phone calls and continuing routine care as usual.
