During the study, patient management details and outcomes will be closely monitored, including brain imaging, blood pressure, glucose levels, and functional status. The primary outcome is functional recovery measured by a utility-weighted modified Rankin Scale score at 180 days. Secondary outcomes include disability, death, and health-related quality of life. The study also collects data on treatments and care decisions to evaluate the Care Bundle's effectiveness and sustainability.

Eligibility Criteria

Eligible

You may qualify if you...

  • Adults (age 2518 years)
  • Diagnosis of spontaneous intracerebral haemorrhage confirmed by non-contrast computerized tomography (NCCT)
  • Admission to hospital within 24 hours of symptom onset or presumed symptom onset
Not Eligible

You will not qualify if you...

  • Previous care limitation
  • End-stage comorbidity with life expectancy less than 6 months (e.g., terminal cancer)
  • Intracerebral haemorrhage caused by brain tumor or cerebral venous thrombosis
  • Clinical signs of brain herniation at first presentation (unresponsive patient with bilaterally fixed, maximally dilated pupils)
  • Pregnant women beyond 22 weeks gestation unless risks and benefits are thoroughly discussed with an obstetrician

Treatment

Duration - Up to hospital discharge or death

Participants receive a Care Bundle involving early intensive blood pressure lowering, reversal of oral anticoagulation, treatment of pyrexia and hyperglycemia, and referral to intensive care or neurosurgery as needed. Participants in the usual care group receive standard clinical management as decided by their treating team.

Continuous monitoring during hospital stay with treatment applied as part of routine care

Follow-up

Duration - Approximately 6 months (180±30 days)

Participants are followed up to assess functional outcomes and quality of life approximately 6 months after intracerebral hemorrhage.

1 to 2 visits depending on follow-up assessments
