Researchers are evaluating a new model for interaction across health services using welfare technology and telemedicine. This model aims to improve how patient information and care routines are exchanged between the municipality and the hospital. It is first being tested for intravenous antibiotic therapy at home and seeks to provide knowledge that can be applied to other conditions, treatments, and areas. The study involves patients who need intravenous antibiotics but are treated at home instead of being admitted to the hospital. They are monitored by the municipal regional response center and nurses following an individual treatment plan, with hospital physicians remaining medically responsible. The approach includes interviews, observations, and data collection from hospital records, alarm systems, and infusion pump logs to develop and assess the service. Participants will be followed over 45 months, during which researchers will measure the number of rehospitalizations, patient satisfaction, and health personnels views on patient safety. Data will be collected from patient records and communication systems. Patients must be trained to manage their infusion pumps and communicate with the response center. The study is designed to ensure safety and quality of home antibiotic treatment through ongoing monitoring and evaluation.
