Eligibility Criteria

- Diagnosed with any form of heart failure
- Age over 18 years
You will not qualify if you...
- Unable to understand the Dutch language
- Allergies that prevent medication uptitration
- Currently admitted to hospice or receiving palliative care
- Severe psychological disorders
