You may qualify if you...

  • 3 years ± 6 months after first cancer therapy completion
  • Received high-risk therapy for first cancer such as alkylating agents, platinums, abdominal or total body radiation, high dose methotrexate, stem cell transplant, nephrectomy, or other therapies possibly causing late kidney or blood pressure effects
Not Eligible

You will not qualify if you...

  • Severe chronic kidney disease or previous kidney transplant before cancer
  • Older than 19 years at 3 years after cancer therapy completion
