You may qualify if you...
- Inpatient liver transplant participants who had blood glucose above 180 mg/dL during hospital stay
You will not qualify if you...
- Planned discharge to a rehabilitation or nursing facility
- Contraindication to sensor placement
- Discharge expected more than 30 days after transplant
- Recent combined transplant including kidney or pancreas
