Who Can Participate
Eligibility Criteria
You may qualify if you...
- Between the ages of 65 and 90 years
- Community-dwelling
- Live within a 50-mile radius of the study site with wireless connectivity
- Have mobility limitations indicated by use of a mobility aid, unsteady gait, or slower walking
- Have cognitive decline indicated by a Mini MoCA Version 2.1 score less than 12
- Have a care partner or adult family member living in the home or nearby
- Willing to wear an activity tracker during the study period
You will not qualify if you...
- Plan to receive skilled rehabilitation services
- Plan to move outside the study area within two months
- Reside in or plan to move to an assisted living or long-term care facility within three months
- Severe vision or hearing loss that interferes with activity or communication
- Unable to stand or walk even with a mobility aid
- Unable to follow a one-step command or hold a conversation by phone
- Unable to commit to the six-week exercise program
- Contraindications to resistance exercise, such as end-stage heart failure
- Have a terminal disease or are on hospice care
- Have a neurological condition affecting motor skills
- Unable to provide consent
