Researchers are studying the effects of a Complete Lifestyle Medicine Intervention Program CLIP for adults with chronic conditions such as prediabetes, type 2 diabetes, pre-hypertension, systemic hypertension, heart disease, dyslipidemia, and excess weight. The program is designed to cover six key areas of lifestyle medicine through 12 to 14 group classes and aims to improve health in these populations. The study is observational and will invite 40 participants over two years, anticipating some will drop out before completion. Participants will attend weekly lifestyle medicine classes for 16 weeks, either virtually or in person, followed by an 8-week weekly exercise program. The program involves collaboration between physicians, health coaches, registered dietitians, and kinesiologists. Monthly sessions with these professionals provide ongoing support. This approach is based on previous research and integrates education and support for lifestyle changes. During the study, participants will complete validated surveys before and after the intervention to assess their lifestyle and well-being. These include the American College of Lifestyle Medicines Lifestyle Assessment Short Form and Well-being Assessment. Demographic information such as age, gender, race, ethnicity, language, education, insurance, and marital status will also be collected. Participant feedback will be gathered at one month and six months to evaluate the programs impact. The study is led by the Northern Ontario School of Medicine and lasts six months.
