Researchers are investigating a lifestyle intervention aimed at preventing non-alcoholic fatty liver disease (NAFLD) and cardiovascular disease (CVD), especially among people with gallstones or who have had gallbladder removal. The study focuses on improving muscle mass, aerobic fitness, body composition, lipid profiles, insulin resistance, and cardiovascular risk through physical activity and nutrition. Previous studies showed a link between gallbladder removal and higher NAFLD risk in men, as well as increased cardiovascular events, motivating this targeted research. The trial will randomly assign 300 participants into two groups: a control group receiving standard lifestyle advice and an intervention group receiving personalized exercise and nutrition programs. The exercise program includes supervised and telehealth muscle-building sessions, twice weekly for six months, followed by six months of telephone support and less frequent supervision. The nutrition program emphasizes an anti-inflammatory diet rich in legumes, low in fat and refined carbs, controlled energy intake based on BMI, extended nighttime fasting, and healthy sleep habits. Participants will be closely monitored throughout the 12-month study, with assessments at the start and after one and two years. Researchers will evaluate changes in NAFLD severity using blood tests and ultrasound, improvements in muscle strength, aerobic capacity, body composition, lipid and insulin measures, cardiovascular risk, body weight among overweight subjects, depression symptoms, quality of life, and lifestyle habits. Adherence to the exercise and diet programs will also be tracked to understand their impact on health outcomes.
