Researchers are evaluating the effect of a mobile application-based medication reminder on how well patients with chronic diseases and memory problems follow their medication routines. The study focuses on patients who face challenges with medication adherence due to memory impairment, which often leads to worse health outcomes and higher healthcare costs. This quasi-experimental study aims to explore how nursing leadership can use digital tools to support better patient self-management and care. Participants will use a customized mobile app that sends automated alerts and reminders tailored to their prescribed medication schedules. The study uses a pretest-posttest design where initial medication adherence and cognitive function are assessed before introducing the app. After using the reminder system for a set period, adherence rates will be measured again to determine any improvements. Training on app installation and use will be provided to participants or their caregivers. During the study, participants will complete baseline assessments including medication adherence and cognitive status. Following app use for three months, follow-up assessments will collect data on medication adherence using self-reports and objective measures like medication possession ratio. Additionally, perceptions of nursing care and leadership support will be evaluated. The total participation time spans from baseline through the three-month post-intervention period, with ongoing monitoring of adherence and cognitive function.
