Researchers are evaluating nutritional management strategies for adults with advanced chronic kidney disease CKD to delay the need for dialysis or renal replacement therapy. The study focuses on the use of low protein diets as a potential way to slow kidney function decline and stabilize metabolic conditions. This implementation study aims to personalize dietary approaches to better meet each patients needs while managing CKD progression. Participants are assigned to one of two groups those who follow a personalized low protein diet with controlled protein intake averaging 0.6 g per kg per day, and those who either refuse or discontinue the diet early, serving as the control group. The study offers a multiple-choice diet system tailored to patient preferences and medical situations to promote adherence to nutritional care. During the four-year study period, participants will undergo diet follow-up and monitoring to assess adherence to the diet program, kidney and patient survival, treatment costs, and inclusion in transplantation or dialysis programs. Researchers will collect data on these outcomes to understand the impact of personalized low protein diets on advanced CKD management. The study excludes individuals with malnutrition, short life expectancy, pregnancy, inability to consent, or lack of healthcare coverage.
