Researchers are investigating how combining leadership alignment with a champion implementation strategy affects the integration of evidence-based mental health treatments in medical care settings in Malawi. The study focuses on addressing the large treatment gap for mental health conditions like anxiety and depression in low- and middle-income countries by testing innovative approaches to better implement these treatments. Leadership support is believed to be a key factor in helping on-the-ground champions successfully promote mental health services. The trial compares two approaches: one using a champion strategy alone, where trained district champions act as change agents with supervision, and another combining this champion strategy with the Leadership and Organizational Change for Implementation (LOCI) approach. LOCI involves engaging district health leaders with data, feedback, coaching, and leadership development to better align organizational priorities and climate in support of mental health integration. Participants are patients aged 18 years or older receiving medical care in participating districts who have screened positive for common mental disorder symptoms recently. Over 12 months, the study will measure how often patients are screened for mental health, assessments for suicidal thoughts, initiation of mental health treatment, adherence to treatment guidelines, and the quality of counseling sessions. These outcomes will help determine the impact of leadership alignment on mental health service integration and guide future implementation efforts in similar settings.
