Researchers are evaluating treatments for men with prostate cancer who have biochemical recurrence after radical prostatectomy. This phase III randomized multicenter trial focuses on improving cancer control by increasing the radiation dose using a hypofractionated schedule. Eligible men have no lymph node or distant metastases and meet specific PSA progression criteria following surgery. Participants will be randomly assigned to one of two groups: the conventional salvage external beam radiation therapy (sEBRT) group receiving 70 Gy in 35 daily fractions over 7 weeks, or the hypofractionated sEBRT group receiving 60 Gy in 20 daily fractions over 4 weeks. Both regimens aim to treat the prostate bed after recurrence, guided by PSMA-PET imaging to exclude metastases. During the study, participants will be closely monitored for cancer progression, with the primary outcome being the 5-year progression-free survival. Assessments include PSA measurements and imaging as needed to track disease status. The study will also evaluate safety, quality of life, and other health outcomes over the follow-up period, which spans at least 5 years post-treatment.
