The trial investigates community-based interventions aimed at improving resilience and adaptive capacity to climate-related hazards in District Badin, Pakistan, a region vulnerable to flooding, cyclones, saline intrusion, and rising temperatures. It compares community-led adaptation strategies and those integrated with primary healthcare engagement against a control group. The study focuses on household-level resilience, health, nutrition, and livelihood outcomes using a multidimensional resilience framework covering social, economic, environmental, and health aspects. Participants receive interventions delivered over 12 months through two models one with Village Management Committees facilitating participatory disaster risk reduction, livelihood, financial planning, and health awareness sessions the other model enhances this by including Lady Health Workers and primary healthcare providers to link communities with health services. Activities include training, group sessions, simulation exercises, and community mobilization to build skills and promote behavior change, with ongoing supervision and monitoring. Participants undergo repeated household surveys at the start and end of the study to assess changes in resilience, vulnerability, food security, mental health, and dietary diversity. Process evaluations monitor intervention delivery and adherence. A rapid assessment will capture responses after any major climate-related event during the study. Data analysis uses statistical models accounting for community clustering and baseline differences. The total participation duration is approximately 12 months.
