Researchers are conducting a randomized controlled trial to compare two postpartum care models among diverse women, including those from racial groups disproportionately affected and rural residents. The study focuses on improving comprehensive postpartum care to ensure timely detection and treatment of complications while addressing patient needs and preferences. It aims to fill important gaps in understanding how best to deliver postpartum care effectively. Participants will be randomly assigned to either the Telehealth Multi-Component Optimal Model (Telehealth MOM) or the enhanced standard of care (ESoC). The Telehealth MOM group receives the enhanced standard of care plus remote blood pressure and temperature monitoring, along with a telehealth visit 10 to 14 days after delivery. The enhanced standard of care group receives education about symptoms, a comprehensive 6-week postpartum visit, and any additional necessary healthcare. Women in the study will complete a comprehensive postpartum visit 6 weeks after delivery. Assessments include monitoring health status through remote devices and follow-up visits to evaluate recovery and identify any complications. The study tracks completion of the postpartum visit as the primary outcome, with a focus on supporting diverse populations through effective care models. Participation spans from pregnancy through 6 weeks postpartum, including education, monitoring, and clinical evaluations.
