Researchers are investigating how to culturally tailor screening and brief interventions for chronic pain and opioid misuse or opioid use disorder (OUD) in clinics serving American Indian and Alaska Native (AI/AN) communities. This study aims to address health disparities and improve care access for AI/AN clients by developing a culturally centered approach that integrates community perspectives and provider training focused on managing chronic pain and enhancing client functioning within cultural wellness frameworks. The study involves implementing culturally adapted strategies for screening and brief intervention in three AI/AN-serving clinics. Providers will receive training to increase their skills and confidence in addressing chronic pain and opioid misuse/OUD through culturally relevant conversations. The trial spans 24 months at each clinical site, using electronic medical record data and staff surveys to evaluate implementation success, alongside patient surveys to assess effectiveness. Participants will be involved through surveys and assessments to understand chronic pain and opioid misuse/OUD among AI/AN clients. Staff perspectives on the implementation process will be collected. The study measures include the number of clients screened during both the implementation and sustainment phases over six months each. The research also monitors safety and treatment needs to inform future care improvements within these communities.
