Older adults aged 75 and older with type 2 diabetes who require insulin therapy face a higher risk of severe hypoglycemia, which can lead to serious complications like hospitalization and death. This research aims to evaluate a safe insulin prescribing approach using an educational program combined with continuous glucose monitoring (CGM) to support these high-risk individuals. This pragmatic randomized clinical trial compares the new strategy to usual care to see if it can reduce severe hypoglycemia episodes and improve patient outcomes. Participants will be randomly assigned to one of two groups: one receiving three group education sessions along with CGM devices, and the other continuing with their usual care without CGM. The education and CGM use aim to help participants better monitor their glucose levels and manage insulin safely. This two-arm study focuses on practical application in real-world settings and targets older adults at increased risk of hypoglycemia. During the study, researchers will measure the occurrence of significant hypoglycemia at 6 and 12 months after starting. They will also assess patient-reported outcomes like diabetes-related distress, fear of hypoglycemia, and confidence in managing their condition using validated surveys. Participants need to be able to communicate in English and access email and the internet to participate. This study will monitor safety and effectiveness over the course of the trial to inform better diabetes care for older adults.
