Actively Recruiting
This research aims to determine the most effective ways to deliver blood pressure cuffs and collect blood pressure logs for self-measured blood pressure SMBP among adult primary care patients with uncontrolled hypertension. The study focuses on improving the implementation of SMBP in routine care settings, especially in underserved populations, by identifying practical and sustainable access and training methods for blood pressure cuffs. Participants will be randomly assigned to one of four groups. Two groups receive a blood pressure cuff by mail, either with an enhanced conversation about returning blood pressure logs or with written instructions. The other two groups receive the cuff in-clinic, again with either enhanced conversation or written instructions on log return. All groups receive standardized components including blood pressure logs, written guidance on taking home readings, and clinic processes for reviewing returned logs. During the study, participants will engage with the assigned cuff delivery and training method, then submit their blood pressure logs as instructed. Researchers measure outcomes such as the rate of cuff receipt, log return, and order completion within 14 days, as well as tracking documentation of blood pressure readings up to 21 days. The trial evaluates workflow barriers, intervention fidelity, and the time taken to complete the intervention, focusing on practical implementation in primary care.
