Researchers are evaluating a Lakota-adapted Family Acceptance Project (LFAP) designed to support Indigenous 2SLGBTQ+ youth aged 13 to 18 and their caregivers. This open pilot trial focuses on assessing the acceptability, feasibility, and safety of the program and research procedures. The study also explores changes in depression, anxiety, and suicidality to ensure outcomes are moving in the expected beneficial direction, within the context of addressing family rejection rooted in colonization and historical trauma. The intervention involves an 8-session group program called Tiwahe Tewichaglapi ("The Family Loves Them"), adapted from the Family Acceptance Project to reflect Lakota culture and history. Sessions are held weekly for 2 hours each and include Lakota cultural and spiritual practices such as smudging, prayer, and shared meals. The program aims to promote caregiver acceptance, strengthen family bonds, increase youth pride and hope, and reduce mental health risks. The study includes two groups where half the families attend sessions first while completing surveys before and after, then the other families complete the sessions after their initial surveys. Participants complete baseline surveys before starting and follow-up surveys immediately after completing the program. Researchers collect data on suicidality, depression, and anxiety from both youth and caregivers at enrollment and after the intervention. The study also includes exit interviews and ongoing input from an advisory board of 2SLGBTQ+ Lakota youth, caregivers, Elders, and professionals to ensure cultural grounding and program impact. Total participation lasts through the 8-week sessions plus survey and interview follow-ups.
