Study of Microbial and Environmental Factors Linked to Polyp Development in Familial Adenomatous Polyposis Patients Undergoing Surgery and Endoscopic Follow-Up

Familial adenomatous polyposis FAP is a genetic disorder caused by a mutation in the APC gene, leading to many adenomas in the colon and duodenum that can develop into colorectal cancer if untreated. This study explores how environmental factors, especially the gut microbiota, may influence the development and severity of adenomas in patients with FAP, focusing on those who have undergone surgery. The goal is to better understand the connection between microbial patterns and polyp development in these patients. Participants are patients with genetically confirmed FAP who have had prophylactic colectomy surgery at least two years ago and are under regular endoscopic surveillance. The study involves collecting fecal samples at home 48 hours before endoscopy, blood samples during hospitalization, and duodenal fluid samples during endoscopy. Additionally, participants complete a food and lifestyle questionnaire. These procedures help researchers analyze microbial composition and environmental factors related to adenoma progression. During the study, participants will undergo their usual upper and lower digestive endoscopies at specific hospitals as part of routine follow-up. Researchers will analyze fecal microbiota composition within 48 hours of endoscopy. Blood and duodenal fluid samples will be collected for further study. The research includes monitoring through questionnaires and reviewing previous endoscopy results. Overall participation depends on regular follow-up visits and sample collection, with no intervention beyond standard care.
